About this role

Under the direction of the Director of Nursing, is responsible for accomplishing the day to day licensed nursing care and related technical work in the care, treatment, and rehabilitation of mentally ill individuals. Under the direct supervision of Nursing Leadership, the Licensed Nurse ensures the safety of the milieu by evaluating medical stabilization, monitoring, assessment and reporting of client needs, crisis intervention, providing structured activities and support. Additional responsibilities include documentation related to daily care, reporting changes to physicians, and assisting with licensed nursing-related duties assigned. The Licensed Nurse will be monitored; under direct management supervision at all times and is responsible to follow through with tasks as trained and as assigned. The Licensed Nurse will be in continual conversation with supervisors regarding client medical care, behavioral stabilization as well as communicate regularly with other members of multidisciplinary team.
The Licensed Nurse position is expected to promote safe, clean, and healthy environment for facility clients. The Licensed Nurse may be required to perform tasks which involve exposure to dangerous equipment and must follow the required procedures for handling, cleaning, or moving of objects/materials and /or cleanup of materials. This position also requires physical capabilities to perform job duties, flexibility in schedule and ability to work in a variety of conditions and multiple environments.
The Licensed Nurse ensures compliance with department protocols, policies, guidelines, and facility standards. 85% of duties include direct client care.

What you bring to the table

Education

• Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) degree from an accredited school of nursing; is a basic requirement for all Licensed Nurse levels.

Experience

• None beyond the basic requirements. Licensed Nurses at this level are expected to perform all duties within the scope of their licensure. However, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.

Licenses/Certifications

• Must possess a current, unrestricted California license as a Licensed Vocational Nurse (L.V.N), Licensed Psychiatric Technician (L.P.T.) or Registered Nurse (RN) license.

What you will do

• Give immunizations; prepare and administer prescribed medications orally, subcutaneously, intramuscularly, rectally, topically, and by inhalation.
• Properly record medications and treatments in the client’s chart; take vital signs; observe, document and report patient reactions to medication and treatments.
• Assist Physicians/Registered Nurses (RNs) with diagnostic tests, treatments and therapeutic measures and observe and record clients’ reactions.
• Assist in the formulation, re-evaluation and adjustment of client care plans to meet physical, mental, emotional and spiritual needs of each patient.
• Observe client’s behavior; identify symptoms which may interfere with the client’s capacity to function; develop good rapport and understanding with the client in order to obtain specific goals; discuss nursing assignments, nursing care, and needs of individual clients with Clinical Staff based on observations and ongoing assessment of client's behavior, and plan therapeutic measures of intervention before a crisis occurs.
• Observe clients' condition, response to treatment and report on progress. Receives physician orders for medications and administers them as prescribed.
• Complete appropriate documentation related to incidents and denial of rights in a timely manner. Facilitate and prepare medication educations groups weekly.
• Assist clients with medication education tests and increasing basic knowledge of medications.
• Assist clients in preparation for out of facility passes, activities of daily living, medication compliance importance etc.
• Communicate with Nurse II, III, or IV to evaluate medication regime, and assist with medication administration of emergency / PRN medications as necessary.
• Assist with admission process to include but not limited to the timely completion of admission paperwork and communicating risk factors to team.
• Assist with client discharge process to include timely completion of discharge paperwork Coordinate and participate in day to day activities for clients such as: Monitoring meals/Diet compliance, Medication compliance, Self-care, Vitals / Temperature, Weights, Mouth Checks, Daily check in with clients, Fresh Air Break Monitoring, Hallway Monitors as appropriate for ensuring medical stabilization and safety, Communicate verbally with appropriate provider during crisis, Verbally follow up with Pharmacy as needed, Complete weekly notes as assigned
• Monitor integrity of charts (storing of, HIPAA, etc.), and auditing for physician signatures COMPLIANCE:
• Understands and adheres to all facility compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Must attend in-service training as assigned by the Nursing department. Direct care staff will be required to complete up to 52hours of in-service training that can be completed outside of facility if pre-approved. Training must include the following topics as outlined in Title 9 & CARF.Relevant CE classes may be used to offset specific topics throughout the year. Uses techniques of Infection Control as outlined in policy and procedure manual and described by Nursing Servicesupervisor(s): Must handle various cleaning solvents, chemicals, etc. Must comply with all regulations such as OSHA, EPA, State Health Department, etc.
