About this role

Under the direction of the Lead Nurse, is responsible for accomplishing the day to day unlicensed nursing care and related technical work in the care, treatment, and rehabilitation of mentally ill individuals. Under the direct supervision of Nursing Leadership, the Client Care Assistant ensures the safety of the milieu by evaluating medical stabilization, monitoring, assessment and report of client needs, crisis intervention, providing structured activities and support. Additional responsibilities include documentation related to daily care, reporting changes to physicians, and assisting with unlicensed nursing-related duties assigned. The Client Care Assistant will be monitored; under direct management supervision at all times and is responsible to follow through with tasks as trained and as assigned. The Client Care Assistant will be in continual conversation with supervisors regarding client medical care, behavioral stabilization as well as communicate regularly with other members of multidisciplinary team.

What you bring to the table

Education

  • Graduate of a Nursing Assistant or Medical Assistant School, or Certification as a Nursing Assistant or Medical Assistant by the state of California is a basic requirement for all Client Care Assistant levels.

Experience

  • Completion of at least 12 months of additional qualifying experience at the Client Care Assistant (level 2) or equivalent experience that has provided the knowledge, skills and abilities necessary to perform satisfactorily the duties of the position may be considered

Licenses/Certifications

  • Current CPR/BLS certification (required)
  • Valid California Driver's License and clean driving history, as determined by insurance underwriting requirements. (Required)

What you will do

KEY JOB RESPONSIBILITIES:

  • Monitors the safety and security of the treatment environment and takes necessary action for client safety.
  • Ensures resolution of client complaints when unresolved at lower levels.
  • Promotes a collaborative model of client care by facilitating communication with nursing and program staff in daily operations.
  • Participates and assists in the daily tasks associated with client care.
  • Participates and assists in the daily tasks associated with admissions and discharge.
  • Admission paperwork and communicating risk factors to team
  • Discharge paperwork completed and timely
  • Assist client with admission and discharge
    Monitors client progress based on the established treatment and care plan(s).
  • Responds promptly in emergency situations to meet client needs.
  • Completes accurate and detailed records of all care provided, ensures charting that is legible, dated and timed.
    Adheres to facility documentation policies in regard to daily summaries, behavioral descriptions, late entries and charting errors.
  • Ensures completion of timely daily summaries of client progress as assigned
    Completes necessary incident reporting paperwork and follow-up.
    Replenish / stock supplies as initiated by supervisor.
  • Assists with client compliance related to care plans and behavioral interventions.
  • Assist client in preparation for out of facility passes, activities of daily living, medication compliance importance etc.
  • Assist with monitoring of clients via hourly, Q15, 1:1, close observation, as assigned. Coordinate and participate in day to day activities for clients.
