About this role

The Social Services Lead is responsible for providing systematic coordination and monitoring of all client admissions and discharge. This position has the overall responsibility with maintaining a stable census in controlling admissions, discharges, and overall milieu as necessary. The Social Services Lead will ensure the processes within the department remain in compliance with all State, Federal and accreditation standards, maintains and submits all collateral information with regards to those standards.This position works with clients, their families, conservators, internal and external stakeholders to address concerns or special needs that may arise during their course of treatment within the facility. This position ensures that documentation necessary for admission is received according to regulations and throughout the clients’ stay; supports the services received and prepares the client for their chosen or appropriate discharge plan.

What you bring to the table

Education

  • Baccalaureate degree in Nursing, or a M.A. or M.S. in Psychology, Social Work, Marriage and Family Therapy, Counseling, or related field (or graduating within 3 months).
  • Successful candidates may be given credit for additional experience in lieu of an education. This exchange is assessed on an individual basis and may be achieved through additional,demonstrated experience in the fields of health administration, psychology, nursing, social work or closely-related fields or positions at the sole discretion of the hiring manager and/or the Human Resource Department.

Experience

  • (2) two years’ experience within a healthcare facility coordinating admission and discharges (preferred).
  • One (1) year of experience in Mental Health highly preferred.

Licenses/Certifications

  • Current CPR/BLS certification (required)
  • Possession of a valid driver’s license as the position may require driving company vehicles. (Preferred)

What you will do

KEY JOB RESPONSIBILITIES:

  • Serve as the contact point and informational resource for clients, treatment team, family/caregiver(s), payers, stakeholders, and community resources as it pertains to admission and discharges.
  • Responsible for receiving referrals and inquiries from hospitals, families, and other referral sources, giving or arranging tours, handling the admissions process according to company procedures. Comply with and maintain current information on state and federal requirements for admission.
  • Conduct pre-admission evaluations whenever necessary either independently or in conjunction with a designated facility representative to ensure appropriate placement of the client, ensuring transparency with client needs, denials, and level requirements on admission.
  • Make person-to-person calls outside the facility to insurance, financial, legal and medical professionals, senior organizations, appropriate special interest groups, assisted living facilities, hospital discharge planners, and other community contacts. Develop and maintain relationships and advise them of bed availability, new products and services, and other pertinent information.
  • Alert appropriate department managers and facility team of projected admissions, bed changes, and discharges. Report information in daily facility meeting. Maintain a close working relationship with the admissions team to assure appropriate and efficient decisions about prospective admissions. When admission determinations are made, work with team on appropriate room and bed selection.
  • Coordinate pre-admission conference.
  • Obtain all necessary medical and legal documents.
  • Acts as an intermediary to Administration and Clinical Specialists on behalf of clients and their families, conservators, providers, and referral sources.
  • Contact community resources and other pertinent sources on facility’s behalf. Provides information about ASTCI, the Harborview area and addresses questions and concerns.
  • Collects data about admission and discharges, prepares reports, analyzes trends, and reports information to appropriate departments for action.
  • Reviews monthly authorization list, follow up that authorizations were received back from optum, file notices of continued stay/extensions, and support the treatment lead with any additional authorization tracking.
  • Authorizations List & Follow up, extensions / treatment lead support, manage insurance authorizations as a whole, payment and understanding of insurance coverage, resources and costs.
  • Maintains an ongoing list of clients and their risk assessment score (RAS) to ensure the client population is appropriately filled and the facility is appropriately staffed.
  • Oversees Discharge Surveys with the client prior to discharging. Oversees follow up phone calls to discharged clients and solicit feedback about their experience and forward any clinical questions to the appropriate manager.
  • Uses findings from analysis of trends in complaints to mentor individuals and departments to develop appropriate service action plans.
  • Triages issues to billing or risk management to ensure better service to our customers.
  • Collects data about client care problems, prepares reports, analyzes trends, and reports information to appropriate departments for action.
  • Manages client court dates through LAC/SDC liaisons, attorney, conservator’s office and treating physician. Schedule transportation / virtual meetings as appropriate.

Maintain a close working relationship with the teams to assure appropriate and efficient decisions about prospective admissions. When admission determinations are made, work with team on appropriate room and bed selection.
