About this role

The Rehabilitation Counselor has overall responsibility for implementing and providing daily services to clients. This is a direct-care position, working closely with clients in a team-based environment, to provide daily care to clients residing on the facility. He/she may work with clients from admission to discharge, following through on all aspects of the client’s residential care on a daily basis to ensure that the treatment plans and structure of program are being implemented by team members on the units.The Rehabilitation Counselor is proficient in crisis de-escalation skills, is attuned to mental health clients and possesses strong interpersonal skills to support a team of other program staff members, as well as interact with other departments. This position is responsible to follow through with tasks, implement interventions outlined by clinical staff and Program Director.The Rehabilitation Counselor will be proficient in all competencies required of the Psychosocial Rehabilitation Specialist positions, however, is expected to perform at superior level specifically related to crisis intervention and providing guidance for staff in their interactions with difficult to work with clients.

 

 

Additional Details

Hours for this position are 3 PM - 11:30 PM to include weekends with Shift Differentials.

What you bring to the table

Education:           

  • Baccalaureate degree from an accredited college or university in Psychology, Social Work, Counseling, Behavioral Sciences, or related filed.
  • Successful candidates may be given credit for additional experience in lieu of an education. This exchange is assessed on an individual basis and may be achieved through additional, demonstrated experience in the fields of counseling, psychology, nursing, social work or closely-related fields or positions at the sole discretion of the hiring manager and/or the Human Resource Department
Experience:     
  • Minimum of (2) two years’ experience working in an acute, long-term and or residential mental health setting. Population experience must include those experiencing psychosis or severe mood dysregulation.
  • Technically proficient in initiating, performing, and completing assigned duties in providing unlicensed program services as validated through advancement/testing procedures.

Licenses/Certifications: 

  • Current CPR/BLS certification (required)
  • Possession of a valid driver’s license as the position may require driving company vehicles. (Preferred)
  • CPI Certification (provided by the facility within first 90 days of employment). Interest in Crisis Intervention Training Certification and ongoing education preferred (CPI facilitator certification)

What you will do

  • Teach and model appropriate social skills.
  • Report changes in client physical conditions/behaviors
  • Ensure resolution of client complaints when unresolved at lower levels
  • Provide services aimed at reducing symptoms/behaviors associated with a mental health disability.
  • Provide interventions and service to promote the highest possible level of success – socially, self-care, increase positive coping skills - through restoration and maintenance
  • Assist clients in educational, work skills, sobriety or with other issues related to their diagnosis by obtaining skills to live independently or by preventing decompensation or a more restrictive situation.
  • Document identifying client symptoms or behaviors and interventions aimed at assisting client with needs.
  • Assist clients in gaining and utilizing skills necessary to participate in community reintegration– educational groups, behavior control, social skills, communication skills, ADLs support, successful interpersonal behavior, and coping skills
  • Observe clients, provide support, positive reinforcement
  • Provide client support in accordance with individualized treatment plans, including ensuring documentation is completed, recorded and physicians are notified timely when necessary.
  • Follows facility documentation policies in regard to daily, weekly summaries, quarterlies, pre- authorizations, behavioral descriptions, late entries, and charting errors.
  • Assisting with crisis de-escalation by communicating with Nurses in identifying client’s current state as well as initiating interventions and safety needs as dictated by client behavior.
  • Focus on prevention of client crisis and incidents relating to program compliance, refusals, symptoms and behaviors.
  • Provides preventative measures daily to decrease escalation of incidents and overall use of seclusions and restraints.
  • Assist clients with program education and increasing their basic knowledge of program goals.
  • Assist clients in preparation for out of facility passes, evaluate client current level of functioning and report / discuss appropriateness with Nurse Leads and Program Leads.
