About this role

The Social Services Assistant Alpine has overall responsibility for working closely with the Social Service Lead and Clinical Specialists to coordinate the admission and discharge of clients and in assisting with appropriate treatment outcomes. This is a direct-care position, working closely with clients in a team-based environment, to provide daily care to clients residing in the facility. The Social Services Assistant Alpine may work with a caseload of clients, following through on all aspects of the client’s care to ensure implementation of treatment plans. In addition, the Social Services Assistant Alpine works with clients, their families, conservators, case managers, internal and external stakeholders to address concerns or special needs that may arise during their course of treatment within the facility. This position works directly with various members of the healthcare team, facility departments, and administration to solve problems that client, families, conservators, internal and external stakeholders may have with the goal of facilitating a positive client experience during their time at the facility. This position acts as an intermediary between facility departments, staff, and the patient/family, conservators, case managers, internal and external stakeholders to bring about the most effective treatment and positive experience.
The Social Services Assistant Alpine ensures compliance with department protocols, policies, guidelines, and facility standards, and assists with the implementation of daily operations, activities, and clinical support of the clients.

What you bring to the table

Education

  • Bachelor’s degree from an accredited college or university in Social Work or closely related field, or relevant experience.

Experience

  • Minimum of (2) two years’ experience with client centered care. One (1) year of experience in Mental Health highly preferred.

Licenses/Certifications

  • Current CPR/BLS certification (required)
  • Possession of a valid driver’s license as the position may require driving company vehicles. (Preferred)
  • Bilingual (Preferred)
  • Registered intern, licensed or licensed eligible as Psychologist, MFT, LCSW, LPCC (Preferred).
  • Completion of the core competencies of the Psychosocial Rehabilitation Specialist (PSRS), Mental Health Rehabilitation Specialist, and/or Clinical Specialist at ASTCI highly preferred.
  • Training or experience with electronic medical records (e.g. Cerner) preferred.

What you will do

KEY JOB RESPONSIBILITIES:

  • Serves as the contact point, advocate, and informational resource for clients, staff, treatment team, family/caregiver(s), payers, case managers, LAC/SDC courthouse representatives and community resources.
  • Establishes a rapport with the client and family to learn what is most important to them in their hospital experience and their preferences. Communicates findings to the appropriate healthcare team members.
  • Works with individuals or departments to limit or resolve delays in service and communicates delays to clients and families.
  • Acts as an intermediary to Administration, Client Placement Coordinator, or Clinical Specialists on behalf of clients and their families.
  • Keeps current on client status information and communicates to family members regarding status, visiting opportunities, and physician meetings. Coordinates information exchange that might facilitate the treatment process.
  • Provides tours/education about the facility to prospective client families. Creates accommodated visits as needed for clients and families through multiple departments to ensure appropriate supervision.
  • Contacts community resources and other pertinent sources on client and/or family's behalf.
  • Completes Discharge Surveys with the client prior to discharging. Makes follow up phone calls to discharged clients and solicits feedback about their experience and forwards any clinical questions to the appropriate manager.
  • Collects data about client care problems, prepares reports, analyzes trends, and reports information to appropriate departments for action.
  • Initiates stakeholder satisfaction surveys twice per year and analyzes data, as assigned by the Program Director.
    Creates monthly authorization list, files notices of continued stay/extensions, and supports the treatment lead with any additional authorization tracking.
  • May testify in court on client’s behavior and responses to conservatorship status.
  • Assists with admission paperwork and communicating risk factors to team. Obtain appropriate financial information prior to the admission based on input from Administrator, Business Office, and Clinical Director.
  • Coordinates pre-admission conference.
  • Monitors inquiry, admissions, and census trends. Maintains Referral Log and other reports as required.
  • Coordinates admissions and admissions paperwork. Obtains signatures from patients and/or responsible parties prior to admission in a manner consistent with state & federal regulations.
  • Participates in various committees and meetings related to department activities and responsibilities.
  • Completes necessary incident reporting paperwork and follow-up.
  • Monitors integrity of charts (storing of, HIPAA etc.)
  • May drive or ride along for client support of off grounds outings, appointments, court appearances as assigned by supervisor.
